Could you explain more on how "introducing a per-visit dollar deductible" violates the condition of accessibility? I think it can be a uniform terms & conditions to all eligible residents?

I'll quote the definition of accessibility from page 10 of https://www.canada.ca/content/dam/hc-sc/migration/hc-sc/hcs-sss/alt_formats/pdf/pubs/cha-ics/2015-cha-lcs-ar-ra-eng.pdf: "to ensure that insured persons in a province or territory have reasonable access to insured hospital, medical and surgical-dental services on uniform terms and conditions, unprecluded or unimpeded, either directly or indirectly, by charges (user charges or extra-billing) or other means (e.g., discrimination on the basis of age, health status or financial circumstances). "
To me, a per-visit dollar deductible falls under an indirect charge.
